GENERAL DESCRIPTION:

Through Tailored Care Management, Behavioral Health Tailored Plan beneficiaries will have a designated care manager supported by a multidisciplinary care team to provide whole-person care management that addresses all their needs, spanning physical health, behavioral health, pharmacy, and unmet health related resource needs.  The Care Management Extender, will assist in meeting the needs of the member, create additional workforce capacity, and allow care managers and supervisors to focus on key tasks for assigned members as well as permit them additional time for members with intensive or complex needs.


WORK DUTIES AND TASKS:

Performing general outreach, engagement, and follow-up with members;

Coordinating services/appointments (e.g., appointment/wellness reminders, arranging transportation);

Engaging in health promotion activities (as defined in the Tailored Care Management Provider Manual) and knowledge sharing;

Sharing information with the care manager and other members of the care team on the member's circumstances;

Providing and tracking referrals and providing information and assistance in obtaining and maintaining community-based resources and social support services;

Participating in case conferences;

Support the care manager in assessing and addressing unmet health-related resource needs;

Regular communication and coordinate efforts with Care Manager and Care Manager Supervisor to decrease duplicative and inappropriate outreach to assigned members;

Complete documentation in a timely manner and as required;

Participate and complete all required agency trainings and meetings, as well as all required care management-based trainings from the State, Technical Assistance entity or payor and within required timeframes as assigned.

QUALIFICATIONS, EDUCATION AND EXPERIENCE REQUIREMENTS

Is at least 21 years of age;

Has a high school diploma or equivalent;

Must be able to pass a criminal background, MVR and Healthcare Registry Check.

Must possess a valid Driver's License.

Must be able to keep strict confidentiality and work with diverse populations.

Is trained in Tailored Care Management;

Is supervised by a care manager and meets one of the below requirements:

Be a person with lived experience with a behavioral health condition and a Certified Peer Support Specialists or

A parent or guardian of an individual with a behavioral health condition and has at least two years of direct experience providing care for and navigating the Medicaid delivery system on behalf of that individual (parent/guardian cannot serve as an extender for their family member) or

Has two years of paid experience performing the types of functions described in the ?Extender Functions? above, with at least one year of paid experience working directly with the Tailored Care Management eligible population.

Individuals who will be able to meet these qualifications, including, but not limited to the following:

Certified Peer Support Specialists;

Community health workers (CHW), defined as individuals who have completed the NC

Community Health Worker Standardized Core Competency Training (NC CHW SCCT);

Individuals who served as Community Navigators prior to the implementation of Tailored Plans; and Parents or guardians of an individual with a behavioral health condition (parent/guardian cannot serve as an extender for their own family member);

A person with lived experience with a behavioral health condition

Physical Demands

While performing the duties of this job, the employee is regularly required to sit, stand, walk, speak and hear.  The position requires extensive computer use so the employee must have sufficient hand dexterity to use a computer keyboard and be capable of reading a computer screen.  The employee must rarely lift and/or move up to 20 pounds.  Reasonable accommodations may be made to enable otherwise qualified individuals with disabilities to perform the essential functions.

Work Environment
