Lead CNA/ Case Manager

GENERAL DESCRIPTION:

The Lead CNA / Case Manager is responsible for supervising and coordinating the Certified Nursing Assistants (CNAs) at the Behavioral Health Urgent Care (BHUC) center. This includes managing CNA schedules, providing direct CNA services, and ensuring quality of care. Additionally, this role provides case management services to clients, linking them to community resources and ensuring continuity of care following their BHUC visit. The Lead CNA / Case Manager works closely with the Program Manager and clinical staff to deliver trauma-informed, person-centered care.

WORK DUTIES AND TASKS:

Job Duties:

25% - Supervise CNA staff, provide leadership, and ensure adherence to protocols

5% - Coordinate CNA work schedules to maintain adequate coverage

20% - Provide direct CNA services including vital signs, observation, and client support

20% - Conduct case management assessments to identify client needs and community resource referrals

10% - Develop individualized service plans and coordinate follow-up care

5% - Maintain documentation of CNA activities and case management services in compliance with Medicaid standards

5% - Participate in team meetings and interdisciplinary planning

QUALIFICATIONS, EDUCATION AND EXPERIENCE REQUIREMENTS:

At least a Batchelor's degree in health or human services required

Active North Carolina CNA certification required.

At least 5 years of experience as a CNA, with at least 3 years in a behavioral health, emergency, or urgent care setting.

Experience in supervision, scheduling, or team coordination strongly preferred.

Knowledge of community resources and case management practices.

Displays initiative in developing action plans and resolving problems as they occur, in consultation with the departmental supervisors and the department Director.

Communication: Maintains effective two-way communication with staff, community agencies, and other audiences using a variety of formats. Examples include: written communications that clearly outline the situation and action items; verbal presentations and briefings to the department Director and members of the management team.

Dependability: Seeks increased responsibility while remaining conscientious, thorough, accurate, and reliable with respect to achieving the organization's goals, and the needs of the IFS consumers and fellow staff. This includes being available and responsive to issues and concerns as they arise

PHYSICAL DEMANDS, WORK ENVIRONMENT, TRAVEL AND MISCELLANEOUS:

Physical Demands

This position requires the ability to respond quickly to high-stress situations, oversee multiple team members across shifts, and manage clinical documentation systems. The Program Manager must be able to lift up to 25 pounds, stand or walk for extended periods, and intervene in behavioral incidents if needed.

Work Environment

Work is performed in a behavioral health urgent care facility that operates 24/7. The environment can be fast-paced and may involve exposure to individuals in crisis, including those experiencing mental health or substance use emergencies.

Travel

Minimal travel required. Occasional travel to community partner agencies, trainings, or off-site meetings may occur.
